Patient Forms

If you are a new patient to our office, please visit our patient forms page to fill out our electronic forms that will be delivered to us online and into your new chart. Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. We also offer payment portals for our patients to make online payments. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

We are a third party to your insurance plan so we can offer estimated information but it is your responsibility to know your dental plan and coverage. We do participate with Metlife, Delta Dental PPO/EPO/Premier, Altus, Aetna, Blue Cross Blue Shield, Cigna, and Guardian along with other smaller insurances as well.
